Oseltamivir (treatment dose) - Dose adjustment for renal impairment.

Cockcroft-Gault CrCl estimates (using the creatinine clearance calculator) should be used for drug dosing rather than the automated MDRD eGFR produced by the clinical chemistry laboratory available on NOTIS.

Oral Treatment doses (Recommended duration 5 days)

30mg as a single dose (remains in body for 10 days)

Haemodialysis (high flux)/ Haemodiafiltration 75mg stat and then 75mg after each HD session
CVVH 75mg od
CAPD 30mg repeated after 1 week  if continuation beyond 5 days is advised by microbiology or infectious diseases (remains in body for 7 days)

Oral Prophylactic dosing (Recommended duration 10 days)

75mg every 48 hours (or 30mg once daily if started in primary care)
